TESTIMONIAL AND PUBLICITY RELEASE

I _________________________ (“Participant”) have voluntarily created a Testimonial for Inner Nature Therapy, Inc. (“Company”).  In consideration of my participation in the services, products and events of Inner Nature Therapy, Inc., I grant Company, as well as its affiliates, agents, employees, licensees or any other party acting on its behalf, the irrevocable, worldwide, unlimited right in perpetuity, to use my Testimonial and Personal Identifying Information, including my name, likeness, image, website, business name, voice, recorded voice, video appearance, biographical information, words and statements, for publicizing and promoting the Company and its services, or for any other lawful purpose.  This grant includes the right to copy, publish, edit, distribute and use in commerce, my Testimonial and Personal Identifying Information in printed publications, multimedia presentations, on websites or any other distribution media, now known or later developed.   I understand that my Testimonial will become the property of the Company and I agree that I will not be compensated in any way for the use of my Testimonial or Personal Identifying Information.  I waive the right to inspect or approve the finished product, wherever my Testimonial and/or Personal Identifying Information appear. 

I agree to hold harmless and release Company, as well as its affiliates, agents, employees, licensees or any other party acting on its behalf, from any and all claims or causes of action arising out of the use of my Testimonial and Personal Identifying Information, including but not limited to, claims for libel, defamation, invasion of privacy, right of publicity, misappropriation or misuse of image.

This Testimonial and Publicity Release is governed by the laws of the state of California and I consent to California as the exclusive venue for any issues arising from it.  If any term of this Testimonial and Publicity Release is found to be invalid, void, or unenforceable under applicable law, the other provisions of shall remain in full force and effect, and shall in no way be affected, impaired, or invalidated.

I am over 18 years of age and I have read, understand and consent to the terms of this Testimonial and Publicity Release.
